Applying for Proctor Appointment
To conduct IMI certification testing, you will need to apply for Appointment as a Proctor
Please download the Proctor Appointment Application Form below. It is available as a downloadable PDF.
Print, fill out and return the form to the IMI International office with a letter from your chapter president, supervisor, or department head, on letterhead stationary, requesting your appointment as a proctor and confirming your qualifications to conduct the exams.
